L\u2019infirmi\u00e8re de garde r\u00e9ceptionne les messages et intervient si n\u00e9cessaire.<\/p><h2><b>Qui peut \u00eatre pris en charge par le SSIAD ? <\/b><\/h2><p>Le SSIAD (Service de Soins Infirmiers \u00e0 Domicile) CLAPA intervient sur prescription m\u00e9dicale au domicile des <b>personnes \u00e2g\u00e9es de plus de 60 ans<\/b><b>.<\/b><\/p><h2><b>\u00a0R\u00f4le du SSIAD:<\/b><\/h2><p>\u25cb Permettre aux personnes accompagn\u00e9es de conserver, d&#8217;am\u00e9liorer, voire de retrouver leur autonomie, afin qu\u2019elles puissent assurer les gestes de la vie courante et r\u00e9pondre \u00e0 une volont\u00e9 toujours affirm\u00e9e de <b>rester dans leur cadre de vie<\/b>.<\/p><p>\u25cb Les soins dispens\u00e9s par le SSIAD permettent d\u2019\u00e9viter une hospitalisation lors de la phase aig\u00fce d\u2019une affection pouvant \u00eatre trait\u00e9e \u00e0 domicile, ou \u00e0 la suite d\u2019une hospitalisation, de pr\u00e9venir ou de retarder l\u2019aggravation de l\u2019\u00e9tat de sant\u00e9 des personnes gr\u00e2ce \u00e0 un personnel dipl\u00f4m\u00e9.<\/p><p><b>\u25cb <\/b><b>Centre de Liaison <\/b><b>: <\/b>pour remplir pleinement son r\u00f4le, le SSIAD CLAPA assure une coordination avec les autres intervenants m\u00e9dicaux et param\u00e9dicaux : Services d&#8217;aide \u00e0 domicile, kin\u00e9sith\u00e9rapeutes, m\u00e9decins traitants, Services hospitaliers\u2026<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-887c789 elementor-widget elementor-widget-spacer\" data-id=\"887c789\" data-element_type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-1d4a631 e-con-full clapa e-flex e-con e-child\" data-id=\"1d4a631\" data-element_type=\"container\" id=\"prise-en-charge\" data-settings=\"{&quot;background_background&quot;:&quot;classic&quot;}\">\n\t\t\t\t<div class=\"elementor-element elementor-element-e66d2c0 elementor-widget elementor-widget-heading\" data-id=\"e66d2c0\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Prise en charge<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-68af4e4 elementor-widget elementor-widget-text-editor\" data-id=\"68af4e4\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><b>La demande peut \u00eatre faite par toute personne soucieuse du bien-\u00eatre de la personne concern\u00e9e : <\/b><\/p><p>La famille, la personne concern\u00e9e elle-m\u00eame, les voisins, le m\u00e9decin traitant, l\u2019assistante sociale, l\u2019infirmi\u00e8re lib\u00e9rale, l\u2019aide \u00e0 domicile, les services hospitaliers,,,<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d7bfbdf elementor-widget elementor-widget-text-editor\" data-id=\"d7bfbdf\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"font-size: 14pt;\"><strong>Documents n\u00e9cessaires<\/strong>\u00a0:<\/span><\/p><ul><li style=\"list-style-type: none;\"><ul><li>Un certificat m\u00e9dical fourni par le SSIAD, \u00e0 faire remplir par le m\u00e9decin traitant.<\/li><li>Si m\u00e9decin hospitalier, le certificat doit \u00eatre renouveler par le m\u00e9decin traitant apr\u00e8s un mois de prise en charge.<\/li><li>Une photocopie de l\u2019attestation de la carte vitale<\/li><li>Une photocopie d\u2019une pi\u00e8ce d\u2019identit\u00e9<\/li><\/ul><\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-1b1d014 elementor-widget elementor-widget-text-editor\" data-id=\"1b1d014\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Avant chaque prise en charge<b>, l\u2019infirmi\u00e8re coordinatrice, <\/b>sur rendez-vous, se d\u00e9place au domicile de la personne ou \u00e0 l\u2019h\u00f4pital, pour recueillir les attentes et propositions de l\u2019usager afin d\u2019\u00e9tablir <b>avec lui <\/b>et en fonction des moyens disponibles un projet individualis\u00e9 de prise en charge pr\u00e9voyant :<\/p><ul><li><b>Des soins d\u2019hygi\u00e8ne, confort et de pr\u00e9vention : <\/b>avec des passages quotidiens ou biquotidiens des aides soignants, selon les n\u00e9cessit\u00e9s et disponibilit\u00e9s. Cela aussi longtemps que l\u2019\u00e9tat de la personne l\u2019exige.<\/li><li><b>Des soins infirmiers <\/b><b>: <\/b>surveillance de la prise de m\u00e9dicaments, de la tension art\u00e9rielle, injections, pansements\u2026<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f63b99b elementor-widget elementor-widget-spacer\" int\u00e9gralement <\/b>par l\u2019Assurance Maladie, quelque soit le niveau de ressources de l\u2019usager,<\/p><p><b><i>Aucune avance de fonds n\u2019est demand\u00e9e dans le respect de la l\u00e9gislation en vigueur. <\/i><\/b><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3da5adf elementor-widget elementor-widget-spacer\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><b>Le suivi <\/b><b>de la prise en charge <\/b>est assur\u00e9 par toute l\u2019\u00e9quipe administrative et m\u00e9dicale<\/p><p>Les \u00e9quipes soignantes se r\u00e9unissent chaque semaine pour \u00e9voquer l\u2019\u00e9tat de sant\u00e9 des usagers.\u00a0 Le cas \u00e9ch\u00e9ant, le SSIAD peut apporter un soutien moral \u00e0 la famille, notamment par des conseils pour l\u2019aider \u00e0 g\u00e9rer le maintien \u00e0 domicile.<\/p><h2><b>Engagement QUALIT\u00c9<\/b><\/h2><p>\u00a0Le SSIAD CLAPA a \u00e9labor\u00e9 un projet de service qui d\u00e9finit ses objectifs en mati\u00e8re de coordination, de coop\u00e9ration, d\u2019\u00e9valuation des activit\u00e9s et de la qualit\u00e9 des prestations ainsi que des modalit\u00e9s d\u2019organisation et de fonctionnement.<\/p><p><b><i>Le S.S.I.A.D a le souci d\u2019offrir \u00e0 l\u2019usager un projet personnalis\u00e9 prenant en compte la personne dans sa globalit\u00e9. <\/i><\/b><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6b73092 elementor-widget__width-inherit elementor-widget
